What does a Health Information Nurse (LPN) do?
n

As a key point of contact for patient inquiries and emergencies, you will utilize your quick thinking and problem-solving skills to triage calls effectively.   • Assists in the coordination of home health services by working with the clinical management or coordinators for all departments, clinical staff, provider offices, hospitals, and all other referral sources. Updates liaisons with hospitalizations of patients as needed.

  • Receives and processes physician's verbal and written orders; documents per process and communicates as necessary to clinical staff or managers.
  • n
  • Receives and processes SBARs; documents per process and communicates as necessary to clinical staff or managers.
  • n
  • Receives and coordinates referrals for all programs including: home care, perinatal, IV, rehabilitation, social service, support services, hospice, community palliative care, and telehealth.
  • n
  • Triages calls; including, referral coordination, physician orders, SBARs
  • n
  • Responsible for participating in CVNA's emergency preparedness processes and emergency building alerts.
  • n
  • Fulfills additional responsibilities as assigned and as necessary, accurately and in a timely fashion.
