1. Prepares non-IV medications for dispensing under the direct supervision of a pharmacist, demonstrating accuracy, precision and consistency relating to selection, packaging and labeling functions.
a. Accurately prepares medications for dispensing from computer fill lists, medication labels or other request forms using accepted policies and procedures.
b. Fills assigned unit dose cassettes and prepares labels as needed in a timely manner to maintain established delivery schedules.
c. Repackages medications according to department policy, maintaining proper documentation and assuring that all work is checked by a pharmacist.
d. Refills medications (including controlled medications) to automated dispensing units. Informs charge nurse as well as pharmacist of all controlled medication inventory discrepancies assuring that all discrepancies are resolved within 48 hours.
e. Screens phone calls and visitors to minimize pharmacist interruptions. Assists the pharmacist by charging for and screening information for missing doses. All completed work is checked by a pharmacist prior to dispensing.
f. Accurately performs C-II safe functions by pulling narcotics for automated dispensing units, patient specific narcotics and refilling EMS kits.
g. Prepares medication code trays as well as other code kits for crash carts according to policy and procedure.


2. Prepares compounded sterile products (IV medication) for dispensing under the direct supervision of a pharmacist, demonstrating accuracy, precision and consistency to selection, preparation, packaging, labeling and maintaining integrity of controlled environments.
a. Performs hand hygiene and garbing in compliance with USP 797 standards and hospital policies and procedures.
b. Able to correctly don required personal protective equipment.
c. Performs cleaning and disinfecting of controlled sterile compounding environment and engineering controls per USP 797 standards and hospital policies and procedures.
d. Accurately performs mathematical calculations related to concentrations, drip rates, and doses.
e. Prepares enhanced sterile product medications including TPN's, control drug drips, epidurals and IVIG according to approved hospital policies and procedures with proper documentation and aseptic technique.
f. Demonstrates competency and understanding of use of primary engineering controls (CAI and LAFW) and secondary engineering controls to compound sterile products aseptically and maintains integrity of controlled environments and lines of demarcation.
g. Prepares and delivers compounded sterile products in a timely manner to maintain established delivery schedules. Consistently meets timelines of preparation within established schedules, policies and guidelines.
h. Utilizes batch records and compounding documentation or software to pull components, noting Lot and Expiration of components for compounding.
i. Labels and stores finished compounded sterile products according to USP 797 standards and hospital policies and procedures.
j. Removes expired or past beyond use dated medications from inventory. Ensures removed medications are quarantined, disposed or destroyed.
k. Recycles sterile products according to hospital policy and procedure.


3. Prepares compounded sterile products (chemotherapy medication) for dispensing under the direct supervision of a pharmacist, demonstrating accuracy, precision and consistency relating to selection, preparation, packaging, labeling and maintaining integrity of controlled environments following USP 800 standards.
a. Performs hand hygiene and garbing in compliance with USP 800 standards and hospital policies and procedures.
b. Able to correctly don required personal protective equipment.
c. Performs cleaning and disinfecting of controlled sterile compounding environment and engineering controls per USP 800 standards and hospital policies and procedures.
d. Accurately performs mathematical calculations related to concentrations, drip rates, and doses specific to the Seacoast Cancer Center.
e. Demonstrates competency and understanding of use of primary engineering controls and secondary engineering controls to compound sterile products aseptically and maintains integrity of controlled environments and lines of demarcation.
f. Prepares compounded sterile products in a timely manner to maintain established delivery schedules. Consistently meets timelines of preparation within established schedules, policies and guidelines.
g. Utilizes batch records and compounding documentation or software to pull components, noting Lot and Expiration of components for compounding.
h. Labels and stores finished compounded sterile products according to USP 800 standards and hospital policies and procedures.
i. Removes expired or past beyond use dated medications from inventory. Ensures removed medications are quarantined, disposed or destroyed.
j. Recycles sterile products according to hospital policy and procedure.
k. Prepares chemotherapy products according to approved hospital policies and procedures with proper documentation and aseptic technique following USP 800 standards.
l. Maintains expertise using closed system transfer device to make chemotherapy preparations.
m. Demonstrates and completes competency with regards to specific calculations including bag volumes, rates and calculations for chemotherapy pumps.
n. Participates, demonstrates and completes mock chemotherapy spill drills.


4. Distributes medications, as required, to patient areas of the hospital.
a. Accurately and efficiently delivers medications to patient care areas according to established delivery schedule, assuring timely provision of medications to patients.
b. Consistently and thoroughly checks patient's medication cassettes at the time of cart exchange to assure all needed medications are transferred to current cassette.


5. Assists in the creation, handling and maintenance of a variety of pharmacy records to promote the efficient operation of the pharmacy and regulatory compliance.
a. Accurately updates patient medication cassettes regarding admissions, discharges and transfers.
b. Accurately and consistently charges and credits patient medications from the cart fill lists.
c. Accurately records quantities of medications issued when filling floor stock requests.


6. Assists in maintaining proper inventory of non-controlled and controlled medications as well as supplies purchased by the pharmacy.
a. Maintains orderly stock levels of all medications in the dispensing area and facilitates the proper and timely filling of medication orders as observed and checked by a pharmacist. This includes IV room stock replenishments.


7. Maintains pharmacy work area in a neat, orderly and clean condition.
a. Medications stored in the dispensing area and work stations (including the IV room) are maintained in a neat and orderly condition at all times, to reduce potential for errors.
b. Outdated medications are removed from stock to prevent dispensing.


8. Actively participates in quality improvement activities of the department.
a. Completes and documents all assigned medication storage area inspections on a monthly basis. Identifies and replaces outdated and unusable medication.
b. Accurately maintains refrigerator temperature logs twice daily to assure proper storage conditions of medications.


9. Maintains competence required for current position description.
a. Maintains current New Hampshire Pharmacy Technician Registration.
b. Maintains current National Pharmacy Technician Certification.
c. Attends pharmacy staff/technician meetings.

Qualifications

Experience Minimum Required
* 1+ year as a hospital pharmacy technician

Experience Preferred/Desired
* 1 year sterile products preparation. Documentation of training in preparing chemotherapy
Education Minimum Required
* HS Diploma
Training Minimum Required
* Chemotherapy Certified Pharmacy Technician
Special Skills Minimum Required
* Pharmacy automation and computer skills. In addition to the responsibilities of a Certified Pharmacy Technician and Sterile Compounding Certified Pharmacy Technician, Certified Oncology Pharmacy Technicians perform routine and specialized technical and clerical duties in the department's sterile product area. Certified Oncology Pharmacy Technicians assist the pharmacist in the operational and clerical requirements in the preparation of antineoplastic agents.
Licensure and/or Certifications Required
* NH Pharmacy Technician Registration required at time of hire.
* National Pharmacy Technician Certification from PTCB within 90 days of hire.
