Key Responsibilities Conduct a Physical Therapy evaluation or screen. Develop an individualized Plan of Care with functional goals. Provide appropriate therapeutic interventions. Evaluate patient response to treatment and modify plan as necessary Document the course of patient care including progress made and the continuing need for treatment. Provide patient and/or family education to help achieve goals. Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others. Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, and Medicare. Assists with orientation of new staff as requested. Meets KHI requirements for skills competency and safety. Participate in continuing education/professional development activities. Assists with the Physical Therapy and Physical Therapy Assistant student program. Participate in staff meetings, client relation and marketing activities. Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy, and Medicare regulatory requirements. Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation. Individual productivity meets department standards. Patient satisfaction will meet or exceed department standards.
Qualifications - Education : Graduate of an APTA approved four year bachelors degree program, or foreign equivalent.
- Licensure: Current PT License through the AZ Board of Physical Therapy
- Certification: BLS and AED through the American Heart Association
- Experience : Recent experience working with an adult and geriatric population
Special Position Requirements Exposure Categories: Category II: Expected duties have possible, but not routine, potential for exposure to blood, body fluids or tissues.
Work Requirements The Incumbent shall have the ability to lift and transfer patients and equipment, have the ability to stand, walk, sit, bend, stoop, crouch, and communicate to provide therapeutic interventions.
