Job Title: Physical Therapist

Guaranteed Hours: 32 hours per week
Experience: New graduates are welcome. Skilled Nursing Facility (SNF) experience preferred.
License: State license required

Job Description:
- Conduct patient assessments and develop individualized treatment plans.
- Provide hands-on physical therapy to patients.
- Work collaboratively with a multidisciplinary team to achieve optimal patient outcomes.
- Maintain accurate and timely documentation of services provided.
- Monitor and evaluate patient progress and adjust treatment plans as necessary.
- Educate patients and their families about exercises and therapeutic activities to aid recovery.
- Ensure compliance with regulations and standards for therapy services.
- Participate in ongoing professional development and training.
Credentialing Requirements:
- State license verification
- Resume
- Three professional references
- County criminal check (7 years for all counties lived and worked)
- Federal criminal check (7 years or more)
- Multi-state criminal search (7 years or more)
- Nationwide sex offender search (7 years or more)
- HHS/OIG search
- GSA search

- Two-step PPD or Quantiferon or proof of positive PPD and chest X-ray
