Certification Details BLS ACLS Job Details Candidates with an AZ home address on their drivers license will not be considered. Please include a copy in submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. Job Requirements 24 months minimum of current ICU experience - REQUIRED Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED No Nursys Hits - REQUIRED Additional Details Flu vaccination - Banner declinations can be applied for (reach out for forms). Please add vaccination status in the submission notes. Banner has very specific background requirements that must be adhered and attested to. Please look at the compliance requirements immediately. Travel experience - Preferred Profile requirements include: Banner Cover Page completely filled out, Resume with all facilities, facility size, beds, trauma level, and units listed, Skills Checklist specific to the discipline (must be less than a year old), Active License (verification must be less than 30 days old), Driver's License. Banner assesses a 2% payment discount fee and a 2% technology fee on all transactions. Learning modules must be completed prior to start of assignment. Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.
