Key Responsibilities Administer and monitor medications with accuracy and safety in skilled nursing Perform vital sign checks and health assessments to ensure effective health Develop and implement personalized care plans with the healthcare Provide patient-centered care, assisting with daily activities and fostering Educate residents and families on health management, wellness, and Document care and maintain accurate patient Apply expertise in medication administration and personalized care Reassess and adapt personalized care plans to evolving resident Conduct routine vital sign checks and update health monitoring Required Qualifications Active RN license in Minimum 1 year of experience in long-term care, skilled nursing, or Expertise in medication administration, vital signs monitoring, and health Strong communication and teamwork Ability to lift 50 lbs and work day shifts (10 :30pm-6:45am ).
