Pride Health is hiring for an RN Case Manager for a client in Kissimmee, FL! (Contract)

Schedule: Mon-Fri (8am-5pm)

Contract: 3 month contract

Pay Rate: $42-46/hour

Primary Responsibilities:

• Works with the providers and clinic staff to identify patients at high risk.
• Perform assessment of health conditions
• Perform medication reconciliation
• Conduct Motivational Interviewing and Self-Management Goal setting
• Provide patient education
• Create referrals to appropriate agencies and resources
• Supports transition of the patient with chronic care conditions from inpatient to outpatient setting
• Perform assessment of transitional needs
• Perform medication reconciliation
• Establish and reviewing contingency plan
• Provide patient education
• Assist with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
• Coordinate with providers to establish or update plan of care
• Performs accurate and timely documentation in the electronic medical record
• Participates in daily huddles and Patient Care Coordination (PCC) meetings

Required Qualifications:

• Active RN license
• Case management experience
• Certified Case Manager (CCM) is a plus
• Current BLS and/or CPR certification
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
