Job Description

The LCSW/LPC provides services to the organization in three broad areas, including services to the patient population, services to increase quality of healthcare planning and delivery, and services to the community to increase the efficient use of healthcare resources. Direct service is provided through the Rural Health Clinics. Direct service includes, but is not limited to: (1) interventions to increase psychosocial functioning; (2) adaptation to illness, injury disability; (3) post-hospitalization planning; (4) psychosocial assessment/evaluation; and (5) facilitation of services such as referral to community agencies, financial planning and provision of information and resource knowledge; (6) interpretation and communication of psychosocial needs to interdisciplinary team; and (7) works as needed with administrative direction to increase resources for patient/families.

Experience and Education:
Minimum:
Master’s Degree from school of Social Work, accredited by the Counsel of Social Work Education or or Master’s degree or higher in counseling, counseling psychology, clinical psychology, or school psychology from a regionally accredited college/university.
1-year post-graduate experience; experience in healthcare setting preferred.
Must have knowledge of systems theory, as well as human and organizational behavior.
Licensure as Licensed Clinical Social Worker or LPC.
Must maintain Behavioral Health certification.
Knowledge/Skills/Abilities Required
Excellent verbal and communication skills as it relates to staff, patients, families, physicians, and allied professionals.
Able to understand meaning of words, ideas associated with them, and their effective use.
Ability to understand instructions, reasoning and make sound judgments.
Provides social service screening and follow-up on patients.
Advise/Consult with family, physician, and other staff concerning needs of patient.
Consult with other clinic patients as directed by the provider.
