Job Title : Licensed Master Social Worker

Location : 50 Water Street, FL 7, Metroplus Health Plan, Manhattan, NY 10004

Duration : 2-3 Months (High Possibility of Extension)

Education : Master’s Degree in Social Work

Shift Details : 09:00 AM-5:00 PM (Hybrid - Travel in the field 1 day a week)

Job Description :

• Experience Required:
• 3 years of prior experience in care management in a health care and/or managed care setting, or other relevant experience working in the field of Social Work or Case Management
• Ability to travel within the service areas including making home visits to members, facility visits to clinical providers, and visits to community, faith, and other social service-based agencies.
• Ability to work closely with members and caregivers.
• Significant experience working with clients around the issues of homelessness, addictions, mental health, entitlements, and housing
• Have a working knowledge of members’ rights and responsibilities as tenants.
• Have experience working on-site in a behavioral health practice setting.

Required Certifications & Licensure:

Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification required.
