LMSW - Social Worker
Location: Brooklyn, NY
Schedule: Mon-Fri, 9AM – 5PM
Salary: $61,000-$72,000
Additional Compensation: $1,000 sign-on bonus and $2,000 retention bonus after one year of employment

We are seeking a dedicated and compassionate Social Worker to join our team. This position plays a critical role in supporting youth and families by collaborating with an interdisciplinary team to assess treatment needs, plan and coordinate services, and provide short-term interventions as necessary. The ideal candidate will uphold our organization’s mission and work to help individuals see their potential.
Key Responsibilities
- Conduct intake and admissions activities in collaboration with other team members.
- Initiate discharge planning and build permanent connections for participants.
- Perform psycho-social evaluations, diagnostic assessments, and coordinate evaluations by other providers.
- Facilitate treatment team meetings, develop treatment plans, and ensure their monitoring and implementation.
- Work with the team and placement services to facilitate transitions to long-term care or discharge.
- Coordinate mental health-related activities and provide crisis intervention services.
- Provide short-term individual and family therapy where appropriate and support behavioral management interventions.
- Facilitate psychoeducation groups for participants.
- Promote permanency through reunification, foster placements, adoption, and alternative living arrangements.
- Maintain proper record-keeping and documentation in compliance with regulatory agencies.
- Implement Sanctuary Model tools and practices to create a trauma-sensitive culture.
- Attend and contribute to internal and external meetings.
- Perform additional duties as required.
Qualifications
- Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
- Minimum of two years of experience in child welfare or a related setting.
- Master’s degree in Social Work required.
- Experience working with youth and families, including crisis management and intervention.
- Strong communication, organizational, and computer skills.
- Ability to work independently and as part of a team.
- Ability to use public transportation and attend off-site meetings or home visits.
- New York State Driver’s License preferred.
- Bilingual in Spanish preferred.
- Must be available to work flexible hours, including evenings, weekends, and holidays, with possible on-call responsibilities.
