Job Title: Long-Term Care Manager

Location: Remote - Hernando County, FL (Member-facing role, with travel required for Home-visits)

Job Description :

We are seeking a compassionate and experienced Long Term Care Manager to manage a caseload of healthcare members with long-term care needs in the Pinellas County area. This is a remote role with 80% of the work being member-facing, involving travel to members' homes or facilities. As part of your responsibilities, you will:

• Conduct monthly and quarterly member contact, including in-person visits and telephonic follow-ups.
• Complete assessments with members, caregivers, and providers to gather information on client status, support systems, and service needs for care plan development.
• Monitor the delivery of services, ensuring follow-up with members, caregivers, or providers to avoid gaps in care.
• Authorize and coordinate referrals for services to meet the members’ needs.
• Collaborate with discharge planners, physicians, and other stakeholders to ensure proper care coordination and seamless discharge and care planning.
• Assist members with filing complaints and appeals to resolve issues efficiently.

Education :

• Bachelor’s degree required.

Required Skills/Experience :

• 2+ years of experience in Care Management or a similar role.
• Familiarity with electronic medical health records.
• Proficiency in Microsoft Office Suite.
• Valid driver’s license and ability to travel to surrounding counties as needed.

Preferred Skills/Experience :

• Experience in home health or long-term care settings.
• Knowledge of discharge planning processes.
• Bilingual in Spanish.

Education :

• Bachelor’s degree required.
