POSITION SUMMARY – RN Case Manager - Hospice

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – CO/compact RN license; BLS; 1 year acute care experience in medical/surgical, ICU, ER or other acute or home health setting experience.

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Either 5-8's or 4-9/hour shifts, with occasional back-up for weekends and on-call.

START DATE – ASAP
