Dates of Coverage: July 5,20,21,24,26,27,31. Aug 2,3,4,7,9,14,15,16,17,18,21,22,23,24,25,28,29,30 Schedule: 8am-5pmScope of Work: Diagnose and treat work-related injuries. Evaluate and treat urgent care patients on a walk-in basis. Requirements: BC, BLS, DEA, active and unrestricted AZ license, MUST have DOT cert, and Covid vaccination.
