The ideal candidate will provide high-quality primary and specialty care, with a focus on LGBTQ+ health. This role requires a commitment to patient-centered care, cultural competency, and a collaborative approach to healthcare delivery.Key Responsibilities:Conducts comprehensive patient assessments, including health history, physical examinations, and diagnostic tests.Develops and implement individualized treatment plans based on patient needs and clinical guidelines.Provides preventive care, health education, and counseling to patients.Manages chronic conditions, including but not limited to, HIV/AIDS, diabetes, and hypertension.Collaborates with interdisciplinary team members, including physicians, nurses, social workers, and other healthcare professionals.Maintains accurate and timely documentation in electronic health records.Participates in quality improvement initiatives and continuing education programs.Qualifications:- Master's or Doctorate degree in Nursing or Physician Assistant from an accredited program.- Current Nurse Practitioner or Physician Assistant license and certification in the state of Illinois- Prior experience in a clinical setting, preferably in primary care or LGBTQ+ health.- Strong knowledge of LGBTQ+ health issues and a commitment to inclusive care.- Excellent communication, interpersonal, and organizational skills.- Ability to work effectively in a fast-paced and dynamic environment.Preferred Qualifications:Experience in community health settings.Knowledge of public health principles and practices.Bilingual in English and SpanishHoward Brown Health is an equal opportunity employer.
