Description
We are seeking a dedicated and dynamic Population Health Pharmacist to join our team. This role focuses on improving patient outcomes through population health outreach, chronic disease state management, and medication therapy management. The ideal candidate will leverage their expertise to optimize medication use, educate patients and healthcare professionals, and implement evidence-based practices to enhance care quality and health outcomes.
Requirements
Key Responsibilities:
1. Population Health Outreach:

  • Develop and execute outreach initiatives targeting specific patient populations to address healthcare disparities and improve access to care.
  • Collaborate with healthcare teams to identify high-risk patients and design interventions aimed at reducing hospitalizations and emergency department visits.
  • Analyze population health data to identify trends and opportunities for improvement.
2. Chronic Disease State Management:
  • Provide comprehensive management of chronic conditions such as diabetes, hypertension, and hyperlipidemia through evidence-based medication use and lifestyle counseling.
  • Monitor and evaluate therapeutic outcomes, adjusting care plans as needed in collaboration with patients and providers.
  • Educate patients on disease states, treatment goals, and self-management strategies.
3. Medication Therapy Management (MTM):
  • Conduct comprehensive medication reviews (CMRs) to assess medication efficacy, adherence, and potential interactions.
  • Develop and implement personalized medication action plans (MAPs) to optimize therapeutic outcomes.
  • Serve as a resource for resolving medication-related issues and improving patient safety.
4. Education and Collaboration:
  • Educate patients, caregivers, and healthcare providers on medication use, adherence strategies, and therapeutic goals.
  • Actively participate in interdisciplinary team meetings and quality improvement initiatives.
5. Program Development and Quality Improvement:
  • Design and implement programs to enhance chronic disease and medication management services.
  • Track and report key performance indicators (KPIs) to measure program effectiveness.
  • Stay current with clinical guidelines, industry trends, and regulatory requirements.
Qualifications:
  • •Doctor of Pharmacy (PharmD) degree required; Board Certification in Ambulatory Care Pharmacy (BCACP) or equivalent preferred.
  • •Active pharmacist license in the state of practice.
  • •Minimum of 2 years' experience in ambulatory care, population health, or MTM preferred.
  • •Strong knowledge of chronic disease state management and evidence-based practices.
  • •Excellent communication, organizational, and analytical skills.
  • •Proficiency in electronic health records (EHRs) and population health management tools.
