COMPENSATION:
- $125,000 to $155,000 Annual Income
- $105,000 to $125,000 Base Salary
- $20,000 to $30,000 Annual RVU Bonus

COMPANY PROFILE:
This organization is dedicated to delivering convenient, high-quality primary care to homebound and facility-based geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.
POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a panel of geriatric patients located in one (1) skilled nursing facility (SNF) in Chillicothe.
Providers see an average of 15 to 16 patients per day.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and adjust treatment plans as necessary
- administer medications, treatments, and other therapeutic interventions as prescribed
- perform minor procedures and wound care as required (and as setting permits)
- monitor vital signs and patient responses to treatment
- educate patients and families on health management, disease prevention, and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate comprehensive care
- ensure compliance with all relevant healthcare regulations and standards
The practice uses the eClinicalWorks EHR.
REQUIREMENTS:
- Ohio NP license
- DEA
- advanced practice experience preferred, not required
- new grads with RN backgrounds in home health, trauma, or ICU are encouraged to apply
