A health services organization in New York City is looking to add a new Case Planner to their team in Brooklyn. In this role, the Case Planner will be responsible for coordinating and monitoring services for clients and is the liaison to all community resources as they relate to discharge and reunification.

- Hours: 9am to 5pm

- Setting: Special Family Foster Care (SFFC) program
- Patient Population: Ages 0 to 25
- Patient Caseload: 12 per worker
- EMR: Connections and NextGen
Responsibilities:
- Create service plans which address safety, behavioral, educational, psychological and permanency needs
- Monitor safety, well-being and progress against other critical outcomes by facilitating regular casework contacts and by documenting the same in progress notes and other case documentation
- Schedule and supervises mandated visits between child and natural family
- Conduct ongoing required interviews in person with child, natural family and foster family
- Independently make all related field visits, to homes, schools, courts, prisons, hospitals and other sites as cases may require
- Attend all Family Court hearings on assigned cases; provide testimony regarding case status and Agency position
- Participate actively in Family Team Conferences and other meetings to advance permanency for children and families
- Ensure that the medical and mental health needs of children and the biological family are being monitored appropriately
- Perform other duties, as needed
Qualifications:
- Bachelor's Degree
- Must have 1 year Case management experience and 1 year Foster Care or Mental Health experience
- Experience in Child Welfare
- Experience with with Mental Health illness affected populations and/or Substance Use Disorder individuals.
- Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV/ AIDS, Substance Abuse, etc.
- Extensive knowledge of Case Management principles and practices
- Microsoft office proficient
- Solid problem solving and time management skills
- Excellent communication skills (written and verbal)
