Once a week they have to go into the office (Usually Tue, Wed and Thur). Mandatory in person
Hours: Branches are open 8am - 5pm, they may have patients who want to be seen before or after.
Homecare Homebase MUST be completed at the patients home.
Absolute must - true home health experience, Oasis documentation
Mileage will be reimbursed at .67/per mile
Join our team in Medford, OR for a 13•week contract, providing home•based care for 20•25 cases per week. This role requires a 40•hour commitment per week, with flexibility for patient needs outside standard hours (8am • 5pm).
Key Responsibilities:
• Deliver home•based care and complete documentation on the same day of service.
• Attend weekly in•person meetings at the office.
Requirements:
• Active RN License.
• 2 years of home health care experience.
• Proficiency in OASIS documentation.
• Experience with Homecare Homebase.
• BLS Certification.
• Continuous employment with no more than a 1•year gap.
Preferred Qualifications:
• Experience with Oasis E (required after Jan 2023).
Note: Time off must be approved in advance and made up. No on•call requirement. Scrub color is flexible. Mileage reimbursed at .67/per mile.
