Position Summary:
Position available in San Francisco County.
Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
Positions also available in San Mateo.
Minimum Qualifications:
Current CA Licensed Vocational Nurse (LVN) license
Current IV Certification
Current CPR card for Healthcare Providers
1 year of Licensed Vocational Nurse (LVN) experience
Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
Demonstrates excellent verbal, written communication, and organization skills.
