* NURSYS report.
* BLS (must be from AHA).
* 2 Supervisory Reference Forms, taken within the last two years
* Driver's license
* Covid Card
* Flu shot.
* EPIC experience
* Active NY license

* The pay rate is up to $75.00 an hour (taxable rate) and the overtime rate of $112.50
