Job Description:
The Practice: Busy private practice providers seeks 1 full time Physician Assistant or Nurse Practitioner to join an established primary care clinic. Our group of providers includes 1 Physician and 3 APPs and is consistently a top quality provider for primary care.
Our practice is a member of the independent practice association (IPA), PHPprime and Colorado Physician Partners. PHP Prime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.
The Position:
- Will work closely with partner physician to manage a panel of primary care patients with a strong emphasis on prevention of illness.
- Orders, administers and analyzes diagnostic tests.
- Performs advanced technical procedures.
- Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
- Maintains accurate, legible, and confidential records.
- Trains patients and families on managing illnesses or injuries and disease prevention.
- Develops and maintains a good working relationship with the healthcare team.
- Approximately 16-18 patients a day.
Qualifications:
- Master of Science degree in Physician Assistant Studies/Master of Science degree in Nursing from an accredited program
- PANCE Certification/ANCC or AANP Certification
- Provider must meet employer credentialing standards
