- ASAP - Ongoing Need
- Shift: Outpatient Coverage: 9am - 4pm. Clinic. Must be available full time - no weekends
- 30% Hematology
- 70% Oncology
- Number of Physicians in Practice: 1 remaining physician
- Hem/Onc only practice
- Adults only
- No Radiation Oncology on-site, but connection to Rad Onc team at another facility, which is 5 miles from practice
- No APP's in practice
- EMR: Epic
- Estimated Credentialing Timeframe: 3 months
- Practice Setting: Outpatient Free-Standing Community Cancer Center
- Average Number of Patients Per Day: 16-20
- Trauma Level II
- Board Certified - Required
- Active IL license - Required
- Local Candidate - Highly Preferred
- Provider that speaks Spanish - Highly preferred
- DEA - Verbal Confirmation Required
