Internal Medicine or Family Medicine Physician
Position Overview:
Seeking a Primary Care Physician to join a comprehensive care program focused on seniors. This role involves in-person assessments, care coordination with an Interdisciplinary Team (IDT), and oversight of participant care in various settings.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans.
- Collaborate with IDT and community physicians for discharge planning and care delivery.
- Oversee care in nursing homes, hospitals, and participant homes.
- Provide direction to Nurse Practitioners and participate in rotating night and weekend call.
Qualifications:
- Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred).
- Active, unrestricted California Physician License, DEA, and CDS certification.
- Eligible for Medicare and Medi-Cal enrollment.
