Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients just north of Providence, Rhode Island.
Opportunity Details: - Full-time, Mon - Fri, 8am-5pm
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
- National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
- Experience in primary care: Internal medicine, geriatrics, or family medicine preferred
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
