- Full scope Optometry services for adults and children
- Site will include co-location with Dentistry, Endocrinology, Behavioral Health, Podiatry, Geriatrics, and Diabetes Nurse Education

- Opportunity for leadership and performance improvement project involvement
- Documentation in eClinical Works electronic health record
- Billing submission at the time of office visit is expected
Requirements:
- New graduates of training programs encouraged to apply
- The Director will also oversee the Patient Engagement Manager, who supervisors the Care Coordinators throughout the Center
- Experience with CLIA, The Joint Commission and NCQA Patient-Centered Medical Home preferred
- Position directly reports to the Chief Medical Officer
