Responsibilities:
Provide personalized, one-on-one physical therapy care to patients in their homes, focusing on helping them regain independence and improve their quality of life.
Collaborate with patients, families, and the care team to develop and implement individualized care plans that align with patient goals.
Participate in a nationally recognized healthcare team by ensuring high-quality physical therapy care and contributing to a supportive, patient-focused environment.
Qualifications:
Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.
Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation
One year experience caring for similar patient population as to be assigned.
