Shift: 1st shift 8:00am-4:00pm

Job Description

· The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines

· Identify resident acuity and needs, help to determine specific care needs, and communicate needs/expectations to families and responsible parties

· Complete assessments, PDPM, and care plans for all residents assigned

· Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines

· Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing

· Ensure accurate, timely completion of the Triggers sheet for assigned residents

· Initiate care plans and supporting activities that will result in the best possible outcome for assigned residents

· Monitor and ensure completion of 7-day look-back documentation by nursing staff within the prescribed time frame

· Maintain current knowledge of PDPM assessment process

· Run the Restorative program in the community

· Responsible for Quality Measure Program

· Other related management duties as directed

Qualifications

Licensed RN in the State of Indiana
One year previous experience in a long term care environment
Certification in both MDS an Restorative
Experience in Restorative Programming; fall care, monitoring, investigation, prevention
Knowledge of Medicare and Medicaid reimbursement
