Job DescriptionJob Title: Physical Therapist

Shift: Day Shift, Monday through Friday, 8:00 am to 4:30 pm

Guaranteed Hours: 40 hours per week

Experience: Minimum of 1 year in an outpatient setting

License: Active state license required

Certifications: Active Basic Life Support (BLS) certification from the American Heart Association

Must-Have:- Experience with outpatient and subacute settings- Willingness to float between outpatient clinics and skilled nursing facilities as needed- Comfortable working independently in a clinic setting

Job Description:- Evaluate and treat patients for physical therapy in outpatient and skilled nursing facility settings as needed- Utilize EPIC navigators for notes and Casamba at the nursing home- Primarily work in outpatient settings with potential cross-training to assist in skilled nursing facilities- Participate in computer-based modules, EPIC training, department orientation, and Casamba training- Adhere to dress code of pewter/gray scrubs and dress casual attire- Opportunity to work in a supportive team environment while maintaining autonomy in certain locations- Facility closed during holidays, allowing for alternate scheduling opportunities
