Key Responsibilities:

• Provide high-quality patient care in the home health setting.
• Assess and evaluate patient needs, developing personalized care plans to achieve optimal outcomes.
• Collaborate with healthcare teams to ensure seamless care transitions and coordination.
• Document patient progress and maintain accurate records.

Requirements:

• Minimum 2 years of experience as a Home Health RN.
• OASIS + HCHB experience required.
• Current RN licensure in state of practice (or compact if applicable).
• Current BLS certification.
• Active Drivers License, vehicle insurance, and access to a dependable vehicle.
