Multicultural Home Care, a Modivcare Personal Care Service, is looking for a Per Diem Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.

• Schedule: Per Diem
• Pay Rate: Paid Per Visit
• Visiting Client Territory: Worcester and surrounding areas
• Office: 250 Commercial St, Worcester, MA 01608
• Pay Rate: Paid Per Visit
HHA Open: $60
PC Intro: $50
HM Open: $40
Quarterly Supervision: $40

You will...

• Performs on-site supervisory and teaching visits to evaluate caregivers.
• Provides orientation to caregivers on assigned home care cases.
• Participates in the training of new nurses and in-services for field staff.
• Performs initial nursing assessments
• Counsels patient/caregiver in meeting nursing and related needs.
• Assigns and coordinates services provided.
• Develop a plan of treatment/care and make necessary revisions based on the patient's condition/needs.
• Prepares clinical progress notes ensuring any changes in the patient's condition are reported.
• Monitors clinical records for complete and accurate documentation.
