Job Title: Hospice RN / RN Case Manager

We are seeking a compassionate and experienced Registered Nurse (RN) to join our hospice team as a Case Manager. This role is ideal for individuals who are passionate about providing patient-centered care and making a meaningful difference in the lives of others.

Key Responsibilities:

• Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load.
• Act as primary liaison between physician, patient/family and hospice team.
• Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity.
• Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.

About You:

• Hospice heart: compassionate, kind and empathetic to your patients and their families.
• Demonstrate flexibility and composure while supporting others in a time of crisis.
• RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
