Outreach PT, OT and SLP is one of the leaders of _*in home*_ *out patient physical, occupational and speech services* in New York and New Jersey. Due to our continued growth , Outreach is looking to bring on additional motivated and passionate Therapists to work with the adult and geriatric populations either in their private home, apartment or in an Assisted Living Facility. *HOME BASED SETTING: PER DIEM to PART TIME PHYSICAL THERAPIST POSITION AVAILABLE IN:*

*Qualifications of Physical Therapist:*
* In Home - Masters degree a minimum requirement
* 1 year of experience working with older adults preferred.
* Home care experience a plus.

*Company Overview: Outreach Rehabilitation*

We are a private practice (not an Agency) solely focusing on Rehabilitation for those that can't make it to an out-patient clinic or opt not to receive Therapy in Clinic setting. Patients may or may not be home-bound, low or high functioning.
* We aim to bridge the gap between a home health agency and an out patient clinic.
* We provide all treatment in patients’ homes, Senior Residences Assisted Living Facilities, or office setting

*Responsibilities of Physical Therapist:*
* We assess patients home environments for safety and environmental barriers
* provide fall risk assessments
* address physical underlying impairments and how it affects overall function
* ambulatory aid and adaptive equipment training
* education to patient and caregiver regarding compensatory strategies and or a Home exercise program

Pay: $60.00 - $74.00 per hour

Pay: $66.00 - $72.00 per hour
