About the Diabetes Center: The Diabetes Center at Barnes-Jewish Hospital is a specialized facility dedicated to the comprehensive care of patients with diabetes. It offers a multidisciplinary approach to diabetes management, providing advanced treatment, education, and support to help patients control their condition effectively. The center collaborates with endocrinologists, dietitians, diabetes educators, and other specialists to offer personalized care plans. Nurse Educator Position: We are seeking a dedicated professional to manage patient schedules for diabetes education and bill for services provided. This role involves working with a diverse patient population, many of whom use CGMs and insulin pumps, requiring expertise in varied technology-based diabetes care. Responsibilities include handling phone calls and in-basket messages, teaching classes, and training patients on diabetes management tools and techniques. Ideal candidates will have strong communication skills and a passion for patient education in a dynamic healthcare environment.

This department covers In-hospital coverage. Hospitalized patients are seen via referral from a licensed medical provider (RN, MD, PA NP).Patients have a diagnosis of Diabetes or are pre- diabetic and need counseling. Often involves the teaching of what Insulin is and the delivery of the Insulin and also other oral agents they may be prescribed. Survival skills are taught to the patient and the patients are instructed to seek out an outpatient program for additional teaching.
Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Monitors patients response and modifies plan of care accordingly.
Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care.
