We are seeking a Social Services Complex Case Manager to:
- Provide case management for patients with complex psychosocial needs and facilitate solutions to the resource needs, behavioral health needs, and psychosocial/environmental issues of patients and families.
- Collaborate with internal and external multi-disciplinary teams.
- Engage patients with the health system and assist team members, patient, and family in overcoming barriers.

Qualifications
- Master's Degree in Psychology, Social Work, Counseling, or other Behavioral Science-related degree Required
Work Experience
- 2 years of case management experience Required
- Case management experience in a behavioral health setting Preferred
Licenses & Certifications
- Valid driver’s license and transportation Preferred
- Case Management Certification Preferred
