We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. 

Highlights
Community Health Network is seeking a team-oriented & skilled Nurse Practitioner with transplant or VAD experience. 

  • Fully integrated advanced program offering all services with the exception of transplantation
  • Chest pain accreditation with 24/7 STEMI coverage, VAD accreditation, structural heart program, impella program, heart failure program, cardio-oncology, pulmonary hypertension
  • Expanding VAD program
  • Electronic Medical Record system (EPIC)

Licensure & Certification

  • Must have current licensure as a Registered Nurse in the State of Indiana
  • Must have current licensure as an Advanced Practice Nurse
  • Must successfully be credentialed by the Network Medical Staff Office

Desired requirements
• Minimum of 3 Years of Transplant or VAD experience 
• Excellent communication skills fostering collaboration with all members of care team
