Job Description: Post-Acute Long-Term Care Nurse Navigator
The Staff Pad has partnered with a client in Montana who has a vision to be the gold standard for Healthcare. We are searching for a Post-Acute Long-Term Care Nurse Navigator to support the organizations mission. This entrepreneurial and inspiring leader will play a crucial role in coordinating patient care within skilled nursing facilities (SNFs) while ensuring the highest quality of service for patients and their families.
Responsibilities:
Advocate for patients and facilitate their journey through the healthcare continuum, improving satisfaction and outcomes.
Travel to local SNF facilities regularly to ensure continuity of care and build strong relationships with care teams.
Collaborate with multidisciplinary teams to create and implement individualized care plans.
Coordinate outpatient and virtual visits, ensuring timely scheduling of appointments and diagnostic procedures.
Maintain accurate documentation in patient EMRs and manage communication between SNF staff and providers.
Educate and support SNF staff on standard operations and care-related topics.
Identify improvement opportunities within care processes and implement strategies to enhance patient experiences.

Requirements
Qualifications:
Minimum of 3-5 years of nursing experience, with at least one year in acute care preferred.
Graduate of an accredited nursing school; BSN preferred.
Current Montana RN license and BLS certification required.
Excellent communication, time management, and collaboration skills.
