  • Typical procedures performed on unit(s): Case Manager provides Case management service for both inpatient and observation patients, drives discharge plan by collaborating with the multidisciplinary team; Social Worker works collaboratively with RN case managers, performs assessments, develops appropriate discharge plans for high-risk patients
