Job Description:
- Provide case management services for both inpatient and observation patients assigned.
- Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients and their families.
- Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary team.
- Evaluate the progression of care using InterQual throughout the episode of care.
- Work collaboratively with RN case managers and the multidisciplinary team.
- Perform assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
- Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
- Maintain the ability to multitask and prioritize workflow and needs.
- Adapt to changes and be a team player.
- Business casual dress code is required.
- Remain calm in crisis situations and deliver information in a clear, concise manner.
- Build trust and act with authenticity.
