Job Type: Per DiemnnTime Type: Part timennWork Shift: Day (United States of America)nnFLSA Status: Non-ExemptnnWhen you join the growing BILH team, you're not just taking a job, you’re making a difference in people’s lives.nnThe CHHA carries out healthcare tasks in the patient’s home under the supervision of a licensed RN or Therapist.nnJob Description:nnHealthcare tasks performed by the CHHA include:nnAssisting patients with appropriate techniques to maintain personal hygiene and grooming, including shower, tub or bed baths and mouth, skin, nail and hair care.nnAssisting patients in the use of toilet facilities, including bed pans.nnAssisting patients in and out of bed using safe transfer techniques, including the use of gait belts and mechanical lifting equipment, when applicable (with appropriate completed/demonstrated skills competency).nnAssisting patients with walking, including the use of walkers, canes and wheelchairs, when applicable.nnPerforming normal range of motion exercises and proper positioning (with appropriate completed/demonstrated skills competency).nnAssisting with cuing of self-administered medications.nnAssisting with meal preparation and feeding, when required.nnObserving, reporting and documenting patient status and care provided.nnTaking and recording of vital signs (temperature, pulse, and respirations) as directed (with appropriate completed/demonstrated skills competency).nnPractice basic infection control procedures.nnDocument status of patient functioning and changes in patient status to be reported to the CHHA’s supervisor.nnMaintaining a clean, safe and healthy environment in the patient’s home.nnRecognizing emergencies and contacting emergency personnel and supervisor when necessary.nnUnderstanding the physical, developmental and emotional needs of the populations served.nnMaintaining adequate fluid and food intake.nnTreating the patient and his/her property with respect.nnCommunication:nnAttends regular case conferences and team meetings per Clinical Manager and as needed.nnCommunicates with other disciplines on the care team and Clinical Manager regarding patient’s status as needed.nnPractices confidentiality principles set by the agency and federal HIPAA guidelines.nnDocumentation:nnAll documentation will be completed in the EMR and in accordance with the agency timeliness policies.nnProfessional Development:nnAdheres to departmental requirements for required training- Attend at least 12 hours of training annually.nnIdentifies specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them.nnOrganizational Requirements:nnHome Health Aides are evaluated for competency in all areas of required CHHA training prior to delivering care to patient. The CHHA will only be allowed to provide care in areas that they have documented proof of competency.nnMaintain strict adherence to the Beth Israel Lahey Health at Home Confidentiality policy.nnIncorporate Beth Israel Lahey Hospital and Medical Center Standards of Behavior and Guiding Principles into daily activitiesnnComply with all Beth Israel Lahey Health at Home Policies.nnComply with behavioral expectations of the Home Care Division.nnMaintain courteous and effective interactions with colleagues and patients.nnDemonstrate an understanding of the job description, performance expectations, and competency assessment.nnDemonstrate a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.nnParticipate in departmental and/or interdepartmental quality improvement activities when needed.nnParticipate in and successfully completes Mandatory Education.nnPerform all other duties as needed or directed to meet the needs of the department.nnAdditional Duties:nnParticipates in the weekend/holiday rotation as defined by Beth Israel Lahey Health at Home.nnParticipates in in-service programs and teaches/precepts other CHHA personnel per Clinical Manager.nnJob Limitations:nnThe home health aide will not function in any manner viewed as the practice of nursing according to the State's Nurse Practice Act. Association of Homemaker and Home Health Aides and possess certificate as evidence of completion.nnLicensure, Certification & Registration:nnMust be a licensed driver with an automobile that is insured in accordance with state.nnHas an active American Health Association BLS. If no American Heart Association BLS the employee must complete an annual BILHAH BLS Competency.
