SUMMARY Coordinates the clinical aspects of the Cardiovascular program, and guides the nursing teams that provide immediate care to Cardiovascular patients as well as coordinating referrals from physicians and health care facilities. Guides patients with cardiac and vascular DRGs through the complex care continuum. Acts as a liaison to the community for Cardiovascular care by providing and organizing education awareness, and prevention and rehabilitation services. Develops educational programs for nurses, clinical staff, physicians, and EMS. Facilitates physician collaboration and utilizes an interdisciplinary approach to Cardiovascular care to improve outcomes in a cost effective manner. Assures patients receive care that consistently follows evidence-based standards and is in accordance with defined regulatory standards. Participates in the collection and monitoring of data for quality improvement of patient care. Completes established competencies for the position within designated introductory period. Other related duties as assigned. The RN Structural heart coordinator(s) in this position is responsible for the care coordination of the structural heart program as part of a team of cardiology providers, APPs, PAs and a structural program support coordinator.
