Under general supervision of the RN and/or Physician, the Licensed Practical Nurse-LPN will support providers in provision of care including recording vital signs, administering methadone, discussing chief health concerns, performing testing, assisting with procedures and documentation.
Assesses, plans, evaluates and delivers nursing care in collaboration with RN and/or Physician.
This work is carried out in support of the mission and goals of Facility
Responsibilities:
Assists with patient care including checking vital signs, blood draws, obtains other specimens for laboratory analysis and culture as ordered by the medical provider, discusses chief health concerns, and administers injections and immunizations.
Observes, measures, records, repots data relating to patient mental and physical status.
Administers methadone under the supervision of the RN.
Prepares labels and take-home bottles of medication for self-administration by client for periods when the medical clinic is closed or for clients who are permitted to self-medicate in accordance with agency policies.
Records and reports any spillage that may occur while dispensing a patients medication.
Participates in the on-going assessment of patients needs.
Follows protocols for methadone dispensing as established by OASAS.
Tallies medication sheet at the end of the day.
Under supervision of an RN physically counts medication at end of day closing.
Interviews patients and record data for admissions or discharge and enters data in computer system.
Follows orders of physician and registered nurse in providing care.
Records and reports patient goals for consideration in a treatment/recovery plan.
Conducts orientation sessions for patients and families.
Reinforces instructions given to patient by RN or Physician and/or assists and participates in health teaching and counseling to patients.
Places phone calls to pharmacies to order prescriptions.
Maintains neatness of work area.
Stocks patient rooms with supplies.
Adheres to safety policies as well as universal precautions.
Initiates emergency assistance as per policies and procedures.
Attends staff development trainings and nursing conferences.
Keeps skills current and increases knowledge and productivity.
Performs other duties as requested.
Requirements:
NYS LPN licensure.
BLS certification.
At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
Available to work a flexible schedule as needed in response to participant and staff needs.
Computer literacy including proficiency in Microsoft Office Suite and EMR.
Ability to put patients at ease and explain physicians instructions.
