A nonprofit health organization in New York City is actively seeking a Licensed Practical Nurse (LPN) to join one of their residential facilities in Brooklyn on a Full-time basis.

Responsibilities: The Residential LPN will:
- Assist the Registered Nurse (RN) in coordinating following through on provisions of healthcare services
- Keep progress notes and summarize for quarterly, semi-annual and annual reviews
- Provide healthcare to individuals receiving services; provide direct hygiene and health instruction to individuals receiving services and families, as required
- Handle control of communicable diseases and infections through identification and assessment
- Provide first aid to persons in need of immediate care
- Assist the Physicians in medical examinations, as needed
- Schedule appointments and follow up on Physicians' recommendations
- Keep up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes
- Administer medications when appropriate.
- Perform other related duties, as requested
Qualifications: - 1+ year of experience as Licensed Practical Nurse (LPN)
- Bachelor's Degree
- NYS Licensed Practical Nurse (LPN)
- CPR certification
- Experience in the delivery of community-based health services to individuals with Developmental Disabilities
- Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
