Licensed Practical Nurse - LPNnnEnglewood, CO ( TypennFull-time, Part-timennDescriptionnnKlarus Home Healthis looking for a full time / part time or PRNLicensed Practical Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receivinghome health services in the Englewood, Aurora, Lakewood and/or Denver metro area.nnThe primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.nnRequirementsnnKey Responsibilities:nnProvide nursing care to patients of all ages in their homes under the supervision of a registered nurse.nnCoordinate care with the interdisciplinary group, patient/family, and referring agency.nnAssist in the development and implementation of patient care plans.nnMonitor patient health and report any changes to the supervising RN.nnAdminister medications and treatments as prescribed.nnEducate patients and their families on health care needs, conditions, and options.nnMaintain accurate and timely documentation of patient care and progress.nnJob Qualifications:nnEducation:Graduate of an accredited School of Vocational/Practical Nursing.nnLicensure:Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.nnExperience:Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.nnEMR system: HCHB experience highly preferrednnSkills:Proficiency in nursing skills as defined by generally accepted standards of practice.
