Job Description Job Title: Nurse Practitioner (NP) - High-Risk Patient Care Position Summary We are seeking a dedicated Nurse Practitioner to play a vital role in reducing the readmission rates for high-risk patients in the Northwest Region. This position involves delivering clinical services in both home and clinic settings, specifically targeting patients with comorbid conditions such as Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD). The NP will take full responsibility for patient care from post-discharge through to sustainable stabilization, ensuring that patients are not at high risk for subsequent readmissions.
Key Responsibilities - Patient Care : Provide comprehensive care to high-risk members either in-home or at the clinic.
- Evidence-Based Practice : Adhere to established clinical guidelines to ensure quality care.
- Documentation : Accurately document all patient interactions and care provided, in compliance with Regal and regulatory standards.
- Follow-Up Coordination : Ensure appropriate follow-up for patients, including access to social and behavioral health services.
- Issue Tracking : Monitor and document clinical and social issues for all patients seen.
- Timeliness : Conduct all visits promptly, following group and health plan protocols.
- Additional Duties : Perform other related tasks as assigned.
Qualifications - Education : Current certification as a Nurse Practitioner.
- Experience :
- Minimum of 3-4 years in healthcare, with specific experience in managing COPD/CHF patients.
- Experience with high-risk, complex patient populations is preferred.
- Home care experience is a plus.
- Skills :
- Excellent verbal and written communication skills in English; bilingual in Spanish is preferred but not required.
- Proficient in Microsoft Office applications.
- Ability to work independently while meeting team deadlines.
Compensation and Benefits The expected salary range for this position is between $130,000 - $140,000 annually, depending on individual qualifications such as market location, experience, and skills.Our comprehensive total compensation package includes:
