Job Description
NYCCC is seeking for a LPN with Phlebotomy experience to provide services at the Bronx Campus and Queens Campus as needed.
JOB duties include:
• Experience in working in medical clinic
• Phlebotomy skills
• EKG
• Vaccination experience, if any
• Filing and inventory
• Making appointments
• Vitals exam/patient triaging
The candidate will primarily work at the Bronx campus, but will also provide coverage in our Queens campus as needed. Our Queens campus is located at 74-03 Commonwealth Blvd, Bellerose, NY 11426.
Work Schedule:
Work days: Monday - Friday
Hours: 8am - 4:30pm
40 hrs/Week
Pass Days Saturdays/Sundays
The selected vendor would be required to complete the mandatory forms prior to interviewing as required by external NYS agencies as well as proof of immunizations including PPD/Tuberculosis testing as well as flu vaccine during flu season or wear a protective mask during flu season.

1) On-Boarding Application
2) Staff Exclusion List Form
3) LDSS-3370 (SCR) Form
4) State Central Child Abuse Register Certification Status Form
5) Health Clearance Form
