We are seeking a highly skilled and compassionate LPN to join our dynamic
healthcare team. As a LPN, you will play a crucial role in evaluating and recording patients'
progress, helping individuals overcome movement limitations, and managing pain effectively. This

Duties will include:
1.Patient Care Management:
• Complete Start of care assessment
• Travel to patients' homes to provide personalized care based on physicians' instructions.
• Develop and implement comprehensive care plans tailored to individual patient needs.
• Administer medication, including insulin, and conduct vital sign assessments such as blood pressure, glucose, urine, and stool tests.
2.Wound Care and Personal Hygiene:
• Inspect wounds, change dressings, and manage personal grooming and hygiene for patients.
• Test for muscle weakness, bedsores, and identify signs of potential infections.
3.Communication and Education:
• Address the concerns and questions of family members, providing clear and empathetic communication.
• Educate caregivers and family members on post-treatment care and ongoing patient needs.
4.Healthcare Advocacy:
• Provide valuable input to physicians and family members for improved healthcare strategies.
• Collaborate with healthcare professionals to enhance patient care and outcomes.
5.Monitoring and Reporting:
• Monitor patient recovery progress and compile detailed reports for physicians.
• Document and maintain accurate records of patient care activities and observations.
6.Continuous Professional Development:
• Stay informed about developments in healthcare through ongoing education, workshops, and attendance at relevant lectures.
• Implement new knowledge and skills into daily practice for continuous improvement.
7.Collaboration with Healthcare Team:
• Collaborate effectively with physicians and other healthcare professionals to develop and adjust healthcare plans.
• Participate in interdisciplinary meetings to ensure comprehensive and coordinated patient care.
Requirements
• Valid nursing license and relevant certifications.
• Proven experience in home healthcare or a similar setting.
• Strong clinical skills with an emphasis on patient-centered care.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a healthcare team.
• Commitment to continuous learning and professional development.
