From our board of directors, staff, volunteers, and donors, the High Peaks Hospice community is filled with compassionate and dedicated individuals committed to providing comfort, peace, and dignity to the families we serve, at a time when its needed most.nnWe help individuals achieve a peaceful life closure, aligned with their values, wishes, choice, and needs, while fully supporting their caregivers and loved ones along the way.nnPosition Overview As a member of the hospice team, the Licensed Practical Nurse, under the direction of the RN Case Manager, is responsible for direct nursing care to Hospice patients and for educating patients and families in home health care procedures.nnKey Areas of ResponsibilitynnMake home visits, under the direct supervision of the RN Case Manager to assist with the provision of care to meet patient/family needs.nnObserve clinical signs and symptoms and report to the RN Case Manager.nnGive personal care to patients as directed by and under the supervision of the RN Case Manager.nnParticipate in the bereavement process as needed and/or appropriate.nnParticipate in team conferences as appropriate.nnComplete all paperwork and report in a timely manner.nnParticipate in continuous education opportunities to provide for professional growth and license certification.nnPerform other duties as assigned.nnQualifications & SkillsnnValid NYS drivers license and available automobile for travel throughout the assigned area.nnCurrently licensed as a Licensed Practical Nurse in The State of New York.nnTwo years experience in nursing is strongly preferred.nnHome Health and/or hospice experience is highly desirable.nnMinimum two years experience.nnMust have a current New York State drivers license.nnMust have 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage.nnAble to cope with emotional stress and be tolerant of individual lifestyles.nnSensitive to the needs of terminally ill patients and families and one's own feelings about dying and death.nnMust meet all local health regulations.nnComputer skills including the ability, after training, to use an electronic medical record system.nnMust be capable of performing the essential functions of this job, with or without reasonable accommodations.nnJob Types: Full-time, Part-time, Per diemnnPay: $20.00 - $25.00 per hournnExpected hours: No more than 37.5 per weeknnBenefits: 401(k) 401(k) matching Dental insurance Flexible schedule Health insurance Life insurance Paid time off Vision insurancennMedical Specialty: Home Health Hospice & Palliative MedicinennSend completed application which can be found at to
