Position Summary: Coordinates and provides rehabilitation/restorative nursing care for residents and provides supervision and guidance to clinical staff members. Insure the concept actively focuses on achieving and maintaining optimal physical, mental and psychosocial functioning. Scope of work may be modified by state specific rules under The Nurse Practice Act.
ESSENTIAL DUTIES AND RESPONSIBILITIES The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. - Supervises, directs and evaluates Restorative C N A's
- Performs treatments for assigned resident's and documents that treatment as required by Regency, state and federal regulations
- Participates in orientation and in-service training for personnel
- Initiates rehabilitation/restorative Functional Maintenance Plan upon discharge from skilled therapy
- Responsible for the weight prevention program with the Dietary Manager and RD, insure prompt implementation of recommendations
- Reports and records pertinent observations and reactions regarding resident's status and any significant changes
- Coordinates nursing care of residents when scheduled for therapy or procedures by other departments
- Assists with or institutes emergency measures for sudden adverse developments in residents
- Initiates restorative nursing when formalized rehabilitation therapy or restorative need arises during the course of a custodial stay
- Ensures' that the RCNA FMP document is within measureable objectives and interventions within the care plan
- Ensure that the resident has the proper equipment to prevent further decline (i.e. positioning & splinting)
- Have a thorough understanding of the policies, procedures, and regulations of the facility
- Assess each new resident for any disability and/or weakness and need for therapy, and also those residents that have concluded therapy services by the facilities therapy consultants
- Determines resident's joint that needs range of motion exercise
- Consult with the resident's physician or physical therapist for limitations or precautions for specific exercises as needed
- Obtain an order from the attending physician for skilled therapy with prompt implements
- Assist with preparation, implementation and continuing evaluation of the resident's care plan quarterly and with any significant change in condition
- Assist in determining the resident's needs and individualized care, if needed
- Give emotional support to the resident and their families, if needed
- Complete the MDS as outlined by the facility policy and procedure
- Attend care plan, Medicare and rehab meetings
- Promotes interpersonal relationships and develops staff morale
- Assures that inventory and supplies are maintained on the unit/hall and communicates need for housekeeping, maintenance and dietary assistance
- Answer resident call lights and ensure that all residents are checked frequently
- Any suspected abuse must be reported to the Director of Nursing/Administrator immediately
- Performs other tasks as assigned
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Educational Requirements and Experience: - Graduate of accredited LPN/LVN School of Nursing
- Valid LPN/LVN license from the State of Alabama
- Valid CPR certification
- One year experience in nursing preferred
