As a Wound Care RN at The Harlem Center, you will play a key role in improving the quality of life, health, and comfort for our residents.
Position Overview:
We are seeking a skilled and compassionate Wound Care RN to join our team at Harlem Center for Nursing and Rehabilitation in Harlem, NY. As a Wound Care RN, you will be responsible for managing and delivering comprehensive wound care services, including prevention and treatment programs, to help optimize resident outcomes. This is an exciting opportunity to contribute directly to the well-being of residents in a dynamic, supportive healthcare environment.
Key Responsibilities:
- Manage and oversee preventive wound care programs and treatment plans for residents.
- Complete detailed wound analysis reports and track resident progress.
- Educate staff, residents, and families on wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies.
- Conduct wound rounds, regularly monitoring residents' wound status and progress.
- Administer prescribed medications and treatments as necessary.
- Communicate any changes in skin or wound conditions to other nursing personnel to ensure timely intervention.
- Perform any additional services or assignments within the scope of practice for a Wound Care RN.
Qualifications: - Valid, current RN license.
- Strong knowledge of nursing practices, procedures, terminology, and regulations, especially those related to wound care and long-term care.
- Exceptional communication, customer service, and interpersonal skills.
- COVID-19 vaccination required.
Preferred: - Wound care certification.
- Previous experience in long-term care settings.
- Familiarity with PointClickCare software.
