MDS Coordinator Cayuga Health System began by joining two longstanding, independent hospitals into a cohesive health-care network providing seamless, comprehensive, community-centered care for the residents of central New York. The MDS Coordinator for Seneca View Nursing Facility under the direction of the Director of Nursing, organizes the completion of the MDS and quality activities to assure compliance with regulatory agencies. Effectively provides Person Centered care and services. Roles and Responsibilities: Assures timely and accurate completion of MDS 3.0 by all disciplines and signs off. Reviews nursing documentation for accuracy and appropriateness and communicates inaccuracies to the Nurse Manager / Director of Nursing. Performs quality improvement activities as determined which may include, but are not limited to: audits, chart reviews, pharmacy reviews, and pressure ulcer monitors. Completes triggers and applies RAPS for care planning in collaboration with the Nurse Managers. Monitors Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement. Determines potential Resource Utilization Groups (RUGs) and expenses associated with a potential admission. Completes and assures the accuracy of the MDS process for all residents. Participates in the admission process of prospective residents in terms of their nursing needs and appropriate placement. Maintains current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicates changes in regulations. This position can work 4-10 hour days or 5-8 with one day remote. Required Skills and Experience: Graduate of an accredited school of nursing with current licensure in New York State; Current NYS Licensed Practical Nurse License. Minimum one year long-term care experience. Onsite training and RAC certification provided. Preferred Skills and Experience: Familiarity with Medicare Documentation and strong knowledge of NYSDOH regulation and the survey process. Physical Requirements: Repetitive use of hands and fingers (e.g., Use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, stooping, and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers); sitting, walking, or standing for long periods of time may be necessary; must be able physically to operate the equipment used for the job. Location and Travel Requirements: Onsite. Pay Range Disclosure: $63,000 to $85,000 per year.
